What a Single Treatment Session Looks Like

A SPRAVATO session takes place in an accredited clinical environment and is directly supervised. This is because the medication can have temporary effects such as feeling isolated, feeling sleepy, or changes in blood pressure.

You start with a check-in and a review of how you’ve been feeling, along with a blood pressure reading. Then you self-administer the nasal spray with the nasal spray devices under the supervision of a health care professional. This typically requires two or three devices and brief gaps between devices to allow the medicine to be absorbed. The spraying itself takes only a short time.

Next, there is the period of observation that has to be done. Staff remain with you for a minimum of 2 hours, monitoring your vital signs (including BP on major sites), side effects, and checking for stability before leaving. Many people sleep in this period; some listen to music and others sit comfortably. Frequent experiences of dreaminess or dissociation are observed, and this usually subsides during the observation period.

Most will be spending 2-2 ½ hours total, beginning from their arrival to their departure. You will require a ride home on each occasion; do not drive at the end of the session, and only drive or operate machinery the next day following a good evening’s restful sleep.

Helpful tip: Arrive a little sooner to your initial visits so you can slowly get comfortable. You might want to bring headphones to be able to listen to music or unwind during monitoring, or you can bring a lightweight blanket. As directed, refrain from eating for about 2 hours prior to the appointment and drink only small amounts of fluids in the few hours before your appointment.

The Typical Course of Treatment: Phases and Timing

Typically, SPRAVATO is combined with an oral antidepressant and is a scheduled treatment consisting of more frequent doses on a short-term basis, followed by less frequent doses as symptoms start to get better.

The induction stage (typically) is the initial 4 weeks. At this time, sessions are two times per week. For many, this is a more robust timeline to allow them an upfront feeling of some relief; for the care team, it allows a timing of potential response and tolerability.

When benefits start, the most typical progression of the schedule is to once a week for the following few weeks. Once these symptoms are well controlled, frequency can stay weekly or be reduced to once every two weeks. The goal is to use the lowest effective frequency, that is, one that will keep the process moving forward.

There is no general time limit. There’s actually a group of individuals who keep maintaining sessions for months and even years if the advantages are still significant. Your provider will monitor and adjust as needed, particularly during the first 4 weeks, and then adjust the plan according to how you are doing.
If it is prescribed solely for depression and has the potential for suicidal ideation or actual suicidal actions, an initial period of twice weekly therapy, for 4 weeks, is also generally considered to offer the best results.
